Job Summary

Fernbrook Family Center is seeking a compassionate and skilled Case Manager / Mental Health Practitioner (MHP) to provide targeted mental health services to children, adults, and families.

In This Role, You Will Support

  • Adults with Serious and Persistent Mental Illness (SPMI)
  • Children with Severe Emotional Disturbance (SED)

You will help clients access critical services—including medical, social, educational, vocational, and financial resources—and provide home and community-based skills services to improve daily functioning under the supervision of a Qualified Mental Health Professional (QMHP).

Key Responsibilities

  • Provide targeted mental health case management services to children, adults, and families
  • Deliver home and community-based mental health skills services
  • Maintain timely, accurate, and compliant documentation
  • Coordinate referrals and collaborate with community providers
  • Meet with clients at least once every 30 days (or more frequently as needed)
  • Facilitate quarterly team meetings for clients and families
  • Participate in discharge planning and care transitions
  • Advocate for clients to reduce barriers and manage crises
  • Attend staff meetings and participate in ongoing training (30 hours every 2 years)
  • Meet productivity standards for client contact and billable services
  • Uphold Fernbrook’s mission, values, and policies
  • Perform other duties as assigned

Qualifications

Required:

  • Bachelor’s degree in a behavioral science field (e.g., social work, psychology, nursing), OR
  • Equivalent experience/training as defined by Minnesota State Statute (e.g., supervised hours, RN experience, or case manager associate experience)
  • Valid driver’s license, reliable transportation, and acceptable driving record
  • Ability to pass a background check

Preferred

  • Experience working with children, adults, and families
  • Case management experience
  • Bilingual abilities
  • Knowledge of community resources
  • Strong assessment and engagement skills

Work Environment & Schedule

  • Flexible hours, typically between 7:00 a.m. – 7:00 p.m.
  • Regular travel to homes, community settings, and offices
  • Full-time: 40–43 hours/week (salaried) or up to 40 hours/week (hourly)
  • Occasional lifting (up to 15 lbs) and extended computer use

Supervision

This role does not supervise staff but requires collaboration with team members sharing client responsibilities.
