Fellowship Village is looking for a motivated and team-oriented full time Social Worker to join our team in Basking Ridge and provide essential support to patients and their families.
To join this team please have the following requirements: Healthcare experience/SW license a must
Qualifications
- CSW/MSW/LSW licensure required
- Minimum 2 years’ experience within a nursing home or rehabilitation center strongly preferred.
- Must be knowledgeable of the rules, regulations, and guidelines that govern nursing care facilities.
Responsibilities for a Social Worker CSW/MSW
- Assists residents with emotional adjustment to facility.
- As directed, interview new residents and records, social history, and assessments.
- Completes the social services portion of the MDS, Care planning, baseline, etc.
- Schedules and attends and participates in interdisciplinary care plan meetings morning meetings, utilization review, etc.
- Writes summaries and progress notes as directed.
- Assists in conducting a pre-discharge evaluation including but not limited to setting up Visiting Nursing Services, resources, etc. in the community.
- Assists with discharge planning. Follows up after discharge.
- Conducts required Cut Letters
- Writes discharge summaries as assigned.
- Develops a professional relationship with each resident and family.
- Educates residents/ representatives on Advanced Directives and POLST.
- Insurance updates and appeals
- Transitions
- Setting up and maintaining the household model
- Adheres to all Social Services Regulations.
