Under the direction of the Director of Care Management (RN), the Licensed Practical Nurse (LPN) Care Coordinator provides comprehensive care coordination services to FellowshipLIFE members and clients across home and community-based settings. The LPN Care Coordinator serves as the primary point of contact for members and families, ensuring seamless coordination of healthcare services, community resources, and interdisciplinary communication to promote safe, high-quality, person-centered care.
The LPN Care Coordinator collaborates with physicians, hospitals, home health agencies, hospice providers, specialists, community partners, and FellowshipLIFE clinical teams to support members throughout the continuum of care. The role emphasizes proactive communication, early identification of health concerns, prevention of avoidable hospitalizations, and exceptional customer service while practicing within the scope of an LPN under RN supervision.

Requirements
- Valid LPN license
- Minimum 1 year nursing experience required
- Experience in community health/home care, desirable
- Valid driver’s license and reliable transportation
