The Memory Care Coordinator is responsible for overseeing the daily operations of the memory care unit within Fellowship Carolina Park. This role ensures the delivery of high-quality, person-centered care to residents with Alzheimer’s disease and other forms of dementia. The Memory Care Coordinator leads and supports care staff, develops engaging and therapeutic programming, and ensures compliance with all regulatory standards.
This position collaborates closely with clinical teams, families, and leadership to create a safe, structured, and enriching environment that promotes residents’ dignity, independence, and overall well-being. The coordinator also plays a key role in staff training, quality assurance, and continuous improvement of memory care services.
Key Responsibilities
- Oversee daily operations of the memory care unit
- Develop and implement individualized care plans and specialized programming
- Supervise, train, and support caregiving staff
- Ensure compliance with company policies and state regulations
- Monitor resident well-being and respond to behavioral or clinical changes
- Communicate effectively with families regarding resident care and updates
- Promote a positive, safe, and structured environment for residents
- Collaborate with nursing, activities, and leadership teams
- Participate in hiring, onboarding, and performance management of staff
Qualifications
Required:
- High school diploma or equivalent
- Minimum of 2–3 years of experience in senior living, memory care, or a related healthcare setting
- Prior supervisory or leadership experience
- Knowledge of dementia care practices and person-centered care approaches
- Strong communication, organization, and leadership skills
- Ability to manage challenging behaviors with compassion and professionalism
- CPR/First Aid certification (or ability to obtain)
Preferred:
- Experience in assisted living or long-term care environment
- Experience with regulatory compliance and state survey processes
- Background in activities programming or therapeutic engagement
