Who we’re looking for?
The Lead Case Manager is responsible for screening the eligibility of clients for the Agency’s Meals on Wheels services. Proficient assessment skills are required to conduct evaluations, provide case management services, and make necessary referrals to appropriate community resources.

What will you do?
- Supervisory responsibility over case manager personnel.
- Department point of contact for external events and meetings that require a representative from the Department to ensure consistent process and assess necessary involvement.
- Consult with Development for grant opportunities that are directed to address client’s support to remain in home.
- Lead consultant between Area Agencies on Aging and Meals on Wheels program to address issues with agency clients that involve safety, security, or well-being.
- Lead in managing, assigning, and reporting on case loads and outcomes for case management personnel.
- Conduct home visits with applicants to determine eligibility and sponsorship level.
- Perform comprehensive social work assessments.
- Provide ongoing case management services including but not limited to managing issues related to service, evaluation of home safety, and assessing need for and providing referrals to appropriate community resources.
- Re-evaluate client’s continued need for services.
- Perform data entry and timely reporting; maintain client records, files, and reports.
- Consultation with Nutrition Services.
- Assist in implementing program evaluation and outcome measurement tools.
- Assist in implementation of strategies to improve outcomes where needed.
- Assist as needed with special programs involving clients and volunteers.
- Provide ongoing support to and collaboration with other FeedMore programs including representation at community events.
- Establish and maintain good community relations.
- Attend staff meetings and other meeting as required.
- Perform duties and maintain standards in accordance with company policies and procedures and State guidelines.
- Other duties as assigned.
Must Have:
- Bachelor’s degree in Social Work or related Human Services plus two years of social work experience working with geriatric and/or other vulnerable populations.
- Must have own automobile and be prepared to use it on the job. Mileage reimbursement is provided for home visits.
- Must have a flexible schedule to accommodate the needs of the agency.
- Must be willing to travel in the Greater Richmond Area.
- Knowledge of the needs of the elderly and persons with disabilities.
- Good knowledge of community resources available to meet client needs.
- Strong interpersonal skills, good time management and organizational skills.
- Ability to show initiative, make independent judgments, and work with minimum supervision.
- Strong oral and written communication skills.
- Ability to gather and successfully assess information for application to real-life situations.
- Knowledge of survey instruments and evaluation tools, and basic statistics.
- Ability to understand and interpret regulations, laws, and state/federal benefits.
- Ability to work with clients, family members and volunteers with empathy and objectivity.
- Ability to be flexible and interact effectively with individuals of diverse backgrounds.
- Ability to secure the cooperation of others.
- Ability to be attentive and accurate, appropriately problem solve and handle crisis situations.
- Proficient in MS Office: i.e. Word, Excel and Outlook.
Ideally, will have:
- Master’s degree in social work.
- Prior experience with non-profit organizations.
- Bilingual in English/Spanish is a plus.
- Three to five years’ experience working with the elderly and/or persons with disabilities.
