What are the responsibilities and job description for the Care in the Community Physician Assistant or Nurse Practitioner position at Fayetteville NC VA Medical Center?
The incumbent works as a Nurse Practitioner (NP) located within the Community Care Service within Fayetteville, NC VA Medical Center. The incumbent provides consult review, training, coordination, management and education related to Referral Coordination Initiative (RCI) and Community Care (CC) program.
Preferred Experience: Working knowledge of community care, Consult management, handling veterans complaints, de-escalating intense discussions
Physical Requirements: The incumbent must be able to perform all activities of the position without restriction. This position may involve standing, walking, stooping, bending, reaching, lifting, and turning. The work may involve going up and down flights of stairs.
- Coordinate care with community providers, specialist, and VA interdisciplinary teams.
- Maintains requirements as outlined in the VHA Office of Integrated Veteran Care (OIVC) Referral Coordination Initiative. Focusing on improving scheduling timeliness and access to care offered to Veterans within VA and community care.
- Demonstrate requirements as outlined in the VHA OIVC Community Care Field Guidebook. Focusing on improving scheduling timeliness and access to care offered to Veterans with community care.
- Provide and produce advanced clinical practices, comprehensive consultations, and management. Carryout knowledge of MISSION Act, COMPACT Act, and other Acts/regulations as it relates to community care eligibility and the use of the Decision Support Tool (DST) to establish Veteran eligibility and the criteria for best medical interest.
- Serves as the clinical consultant with community care providers and the third-party administrator to determine clinical necessity and need when guidelines are not in place.
- Apply a complete review of medical documentation for compliance with established VA guidelines, directives, and local policies.
- Oversee and update medical records to reflect accurate and current status.
- Maintain confidentiality of electronic, written, and/or verbal patient/employee information.
- Demonstrate working practice that includes adherence to practice standards and the safe use and operation of equipment.
- Other duties as assigned.
- Communicate and interact appropriately and courteously with all internal and external customers.
- Maintain confidentiality of electronic, written and/or verbal patient/employee information.
- Practice to the full extent of their education, training, and certification. Core functions are outlined in the clinical privileges that correspond with this position.
- Utilize Windows, CPRS, VistA, CTB, DST, and Microsoft Office software.

Supervisory Controls: The incumbent reports to the Community Care Medical Officer. The employee independently plans and carries out the assignments, makes day to day sound decisions using their own initiative, consult on the complex problems to their supervisor as needed. The incumbent is responsible for the quality and technical accuracy of the work performed. The employee, having developed proficiency in the area of work assigned is responsible for the planning and carrying out the assignment, resolve conflicts that may arise and select the approach used in solving the problem. Complete work is reviewed based upon results and meet specific program goals and objectives. The employee notifies the supervisor of project progress and any potential controversial matters

Qualifications:
Advanced Practice Nurse (Nurse Practitioner) Basic Requirements- Education: A post-master's certificate or master's or doctoral degree as a Nurse Practitioner. (Applicants pending the completion of educational requirements may be referred and tentatively selected but may not be hired until the education requirements are met.) Must be a graduate of a school of professional nursing approved by the appropriate accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant:
- The Accreditation Commission for Education in Nursing (ACEN) or
- The Commission on Collegiate Nursing Education (CCNE).
- NOTE: Foreign Education. Graduates of foreign schools of professional nursing programs, the nurse practitioner degree equivalency must be validated through an Agency recognized degree equivalency evaluation agency, such as the International Consultants of Delaware (ICD). Candidates with equivalent nurse practitioner masters or doctorate level degrees who possess current, full, active, and unrestricted registration and certification will meet the requirement of graduation from an approved school of professional nursing to warrant appointment.
- License and Certification:
- License. All APN (NPs) must possess and maintain an active, current, full, and unrestricted APN license to practice as an NP in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
- Basic Certification. All APN (NPs) must maintain full and current board certification in an area of clinical specialty from one of the following: the American Nurses Credentialing Center (ANCC), American Academy of Nurse Practitioners Certification Board (AANPCB), or other nationally recognized certifying body in the area in which the Nurse Practitioner is academically prepared.
