The Role
Farm to Fork Meals is seeking a results-driven Manager of Healthcare Partnerships to expand our presence in the Managed Medicaid and Healthcare Partnerships space.
This is a non-traditional sales and relationship management role ideal for someone passionate about bridging the gap between nutrition and care coordination.
The position focuses on building and maintaining partnerships with managed care organizations, case managers, and community agencies to support individuals receiving post-discharge meal delivery, chronic care nutrition support, and Long-Term Services & Supports (LTSS). Farm to Fork Meals also proudly serves children, military personnel, and emergency responders through tailored nutrition programs.

· Present Farm to Fork Meals’ products and nutrition-based programs to managed care organizations, case managers, social workers, physicians, nurses, and dietitians.
· Build and nurture relationships with healthcare partners and referral sources to expand program participation.
· Plan and execute weekly schedules for virtual and in-person meetings, educational events, and drop-ins.
· Coordinate staff meetings, in-service trainings, and healthcare conferences.
· Support and educate referral sources on enrollment and service processes.
· Manage and track referrals, including source, volume, and outcomes, using the company CRM.
· Provide monthly performance reports on referrals, utilization, and outcomes.
· Conduct regular outreach (20–30 calls/emails per day) and quarterly in-person or virtual visits/in-services (2–3 per week).
· Identify and resolve issues with referrals, deliveries, or authorizations.
· Partner with leadership to define target markets and execute growth strategies.
· Travel up to 50% within the assigned state or territory.
· Bachelor’s degree in Business, Marketing, Healthcare Administration, or a related field. the
· Minimum of 4 years of experience in healthcare sales, case management, or related fields such as managed care, long-term care, home health, or medical equipment sales.
· Excellent communication and presentation skills with the ability to engage diverse healthcare audiences.
· Proficient in Microsoft Office 365 and CRM systems.
· Strong attention to detail, organizational, and documentation skills.
· Results-oriented mindset with the ability to work independently and collaboratively.
· Passion for community health, wellness, and improving access to nutritious meals.
This position involves frequent standing, walking, driving, and travel between client or partner locations. Candidates should be able to sit for extended periods, lift up to 25 lbs., and work both indoors and outdoors as needed.
