What are the responsibilities and job description for the Family Support Specialist position at Family Tree Relief Nursery?
Assists families that are referred by the Department of Human Services Self Sufficiency and community referrals, to remove barriers to improve family stability with the purpose of increasing family self-sufficiency. Assistance includes informing families of community resources and supporting accessing services by attending appointments and completing documentation. Families are also supported with parent education, both in-home and with referrals to other resources.
ESSENTIAL DUTIES AND RESPONSIBILITIES
90% Engage with referred families
- Accept and initiate contact within 24 hours for all referrals made by DHS Family Coaches and the community.
- Schedule an Intake with the family, considering the cultural and socio-economic background, and complete Intake documentation.
- Work with the family to set family goals, completing and managing all documentation and the client file in accordance with Family Tree Relief Nursery Case Management and DHS requirements.
- Provide comprehensive bi-weekly in-person outreach services ranging from home visits to community support to assist families to achieve goals and access relevant community services.
- Maintain frequent oral and written contact with community partners, discussing as appropriate all accomplishments and any concerns regarding progress, and document accordingly.
- Provide in-home parent education
OTHER DUTIES AND RESPONSIBILITIES
10% Other Duties
- Support Front Office Coordinator with answering phones, greeting visitors as needed.
- Present in Court as required.
