Family Ties' Every Kid Counts is a voluntary, short-term, early prevention/intervention program that serves low and moderate risk families referred by the Westchester County Department of Social Services. The goal of the program is to mitigate risk, strengthen a caregiver’s protective factors, and prevent a family’s deepening involvement in the Child Welfare system, including Mandated Services and/or out-of-home placement.
The Bilingual Case Planner works in partnership with families throughout Westchester County and provides services in-home, as well as in the community at times convenient for the family. Program services include evidence-based assessments, parent coaching using an evidence-based model, advocacy, support, and community referrals.
Full-Time (35 hours a week). Occasional evening and weekends required.
Salary: $59,000 per year
Responsibilities
- Manage a caseload of 6–8 families.
- Maintain weekly contact with families at times/locations convenient for them.
- Conduct assessments using CANS and FRIENDS tools; develop service plans collaboratively.
- Monitor safety and risk throughout family involvement.
- Provide parent coaching using the Triple P Parenting model.
- Advocate for families in meetings, appointments, and Family Court as needed.
- Facilitate Wraparound meetings and connect families to community resources.
- Attend staff meetings, trainings, and partner events.
- Complete all documentation in compliance with agency and funding requirements.
- Participate in agency quality improvement initiatives.
Qualifications
- A minimum of a Bachelor of Social Work degree, or a bachelor’s degree in a related field plus two years’ experience working directly with families providing social work and case planning/management services.
- Excellent verbal and written communication skills in both English and Spanish.
- Flexibility to work some evenings and weekends.
- Proficiency in MS Office Suite and data management systems.
- Solid record keeping and documentation skills.
- Superior time management and organizational skills, with the ability to meet deadlines.
- Ability to work with diverse racial, cultural, and economic groups.
- Valid NYS driver's license, safe driving history, and a vehicle that is properly insured and registered in accordance with NYS DMV regulations.
- Satisfactory clearances from the Child Abuse Registry and Criminal History Information Tracking System.
