Caregiver Thousand Oaks Monday and Wednesday shifts from 10:30 to 4:30

Introduction:

Family Ties Home Care has a client in Thousand Oaks with moderate Dementia who needs help during the day with walking and being engaged giving his wife respite to get things done. He loves to go on walks and must be watched at all times with that. Likes to engage with CG and have active conversations and activities . We want to encourage him to get out and possibly socialize or go to different parks for walks.

Responsibilities:

• Following a pre-set care plan
• Standard ADLs (personal care, bathing, dressing, grooming, hygiene assistance)
• Assist with medication reminders
• Monitor and report on client's well-being
• Light housekeeping, laundry, & other household daily chores as needed
• Grocery shopping & errands
• Cooking/meal preparation
• Scheduling appointments, including doctor's
• Communication with Family Ties HC management & family members/loved-ones regarding the client's daily status
• Daily notes
• Other duties assigned as needed

Qualifications:

• Current HCA or higher certification
• Caregiving experience of 1 year or similar experience
• Compassionate and caring demeanor
• Excellent communication & interpersonal skills
• Ability to follow the pre-set care plan and medication schedule
• Ability to lift 35 pounds
• Ability to assist with client's mobility as needed
• Valid driver's license and reliable transportation
• Ability to work the schedule as described
