FSW’s Community Residence for Children and Adolescents is an OMH-licensed eight-bed residential setting for adolescent boys who have been diagnosed with a Severe Emotional Disturbance (SED).
JOB SUMMARY
Family Services of Westchester is seeking a Full-Time Case Manager to join our Community Residence Program in New Rochelle, NY.
RESPONSIBILITES
- Provide case management services including casework, individual and group counseling, and other services outlined in the individualized treatment plan for each resident and their families
- Responsible for writing annual treatment plans, quarterly service plans, monthly Medicaid billable progress notes, and psychosocial summaries when indicated.
- Facilitate weekly treatment team meetings to review resident's needs and behaviors, community residence procedures, and to follow through on each issue. Maintain written summaries of the weekly treatment team meetings.
- Engage and work with families in preparing for their child's discharge home; Appear at court hearings as an advocate for residents; Share on-call coverage responsibility.
- Responsible for being informed of critical incidents, management problems, and the handling of same, with support to the Program Director.
QUALIFICATIONS
EDUCATION
- Masters Degree in Social Work -MSW-(Required)
EXPERIENCE
- 1 years of experience of supervised social work experience in a recognized community based mental health or psychiatric hospital setting.
- Experience with clinical casework and group counseling with adolescents and their families.
LICENSE
- Valid NYS Drivers License- (Required)
