- Conduct thorough patient assessments, including physical needs, physiological evaluations, and medical history reviews, to determine individual care needs.
- Collaborating with multidisciplinary teams across hospital, clinic, and post-acute settings.
- Develop and implement personalized plan of cares that incorporate post-acute care needs and community resources.
- Manage utilization review processes by evaluating diagnostic evaluations, medical records, and treatment plans to optimize patient outcomes and resource use.
- Facilitate intake procedures for new patients, ensuring compliance with HIPAA regulations and accurate documentation in medical records for state compliance.
- Provide case management support for diverse populations including pediatrics, geriatrics, behavioral health patients, and those with infectious diseases or trauma-related conditions.
- Engage in patient education initiatives focused on medical terminology comprehension, medication adherence, and self-care strategies to promote recovery and well-being.
- Strong knowledge of physiology, diagnostic evaluation processes, and inpatient versus outpatient workflows supports effective case management.
- Familiarity with managed care principles, utilization management protocols, and healthcare regulations such as HIPAA ensures compliance and quality assurance.
