We are seeking a Medical Case Manager to support individuals living with HIV/AIDS by building strong relationships, coordinating care, and helping clients access treatment and essential services.
What You’ll Do
- Provide case management and advocacy for individuals with HIV/AIDS
- Conduct intakes, assessments, and develop client-centered care plans
- Educate clients on HIV prevention and treatment adherence
- Connect clients to medical care, housing, and social services
- Conduct home, hospital, clinic, and community visits
- Collaborate with providers on care and discharge planning
Qualifications
- Bachelor’s degree in Social Work or related field
- Bilingual (Spanish/English) preferred
- Strong communication and relationship-building skills
- Valid driver’s license, reliable vehicle, and insurance required
Schedule
Primarily daytime hours with occasional evenings as needed.
