Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.SCHEDULE:Program Hours: Monday-Friday 9am-5pmFlexible schedule and hours24 hours weekly SUMMARYFamily Service League is seeking a part-time Psychiatric Nurse Practitioner for the Assertive Community Treatment (ACT) program, which provides behavioral health services to adults who have been diagnosed with a severe and persistent mental illness. The ACT program is comprised of a multi-disciplined team including skilled staff in psychiatry, nursing, and social work. The Psychiatric Nurse Practitioner will be given an agency vehicle to use for ongoing home visits with the clients. Travel is required. RESPONSIBILITIESThe Psychiatric Nurse Practitioner will prescribe and revise medical regimens.Interpret findings and plans of care according to collaborative practice agreements and protocols in designated areas.Administer and monitor medications, including injectable medication.The Psychiatric Nurse Practitioner will provide medical assessments and vitals for consumers in the community.Conduct health assessments of adults with potential, acute, and/or chronic health problems.Initiate referrals to specialty services and foster continuity of care.The Psychiatric Nurse Practitioner will initiate selected therapeutic and diagnostic procedures, consultation, and laboratory examinations.Provide patients and their family members with education and counseling in the areas of health promotion, maintenance, and disease prevention.Participate in team meetings and conferences to enhance an interdisciplinary approach to primary health care delivery.The Psychiatric Nurse Practitioner will serve as liaison between the patient and the community, and act as a health care advocate.Identify quality improvement issues and document standards of care and practice.Maintain a current level of knowledge relative to professional practice.Act as a resource to staff and implement educational programs.Participate in the evaluation of the cost effectiveness of services provided.All other duties as assigned. QUALIFICATIONSMaster's Degree from a Psychiatric Nurse Practitioner Program required.Current license to practice as a Registered Professional Nurse and a Psychiatric Nurse Practitioner in New York State required. National Board Psychiatric Certification.Candidate must possess a collaborative practice agreement with a physician and designated protocols, both filed with the New York State Department of Education. At least 1 year of clinical experience in a behavioral health setting required. Proficient computer skills, including Microsoft Office required; experience with an EMR is preferred. Excellent time management, interpersonal, and verbal and written communication skills required.Valid and clean New York State Driver’s License required.PHYSICAL REQUIREMENTS This position requires sitting for long periods of time and travelling to conduct home visits.
