FAMILY SUPPORT SPECIALIST

Established in 1926, Family Service League is a social services agency transforming lives and communities through mental health, addiction, housing, and essential human services with more than 60 social service programs in over 20 locations.SCHEDULE:Monday-Friday 9AM-5PM Pre-scheduled occasional weekends or evenings required.SUMMARY:Family Service League is seeking a full-time Case Manager for the Smart Start program. The Case Manager will visit families twice a week in a pre-screened home setting to model and reinforce positive interaction between the child and parent/caregiver. The Case Manager will work with families, caregivers, and communities to support early literacy, school readiness and early opportunities. The Case Manager will be handling cases in the Bellport and/or Wyandanch communities. Travel is required. Must be able to drive personal vehicle to and from home visits.We offer a generous benefits package including the following:Health and Dental InsuranceVision InsuranceFlexible Spending Account (FSA)Retirement Savings Plan with a 5% employer contributionLife and AD&D InsuranceGenerous PTO (paid time off)Up to 11 paid HolidaysPaid Sick LeaveStudent Tuition Remission ProgramEmployee Assistance Program (EAP)Company paid Short-Term and Long-Term DisabilityEmployee Discounts and more!RESPONSIBILITIESThe Case Manager will complete a minimum of 16 hours of training delivered by the Supervisor prior to beginning home visits.Visit with families in the family's home on a twice weekly basis, demonstrating reading, play and verbal interaction behaviors with two-year-old and three-year-old children and the parent or primary caregiver.The Case Manager will complete the required paperwork on a weekly basis.Attend and participate in weekly supervisory meetings, staff meetings, individual supervision and training, as is required.All other duties as assigned.QUALIFICATIONSHigh school diploma or equivalent and 5 years of experience working in human services required. Bachelor’s degree in human services or related field preferred.At least 1 year of prior experience working with young children and parents in a community-based organization is required.Must have demonstrated knowledge of the community being served.Proficient computer skills, including Microsoft Office, required.Excellent interpersonal and communication skills, both verbal and written, required.Ability to work independently and maintain confidentiality required.Strong judgement and problem-solving skills, including knowing when to seek help, required.Bilingual in Spanish preferred.Valid and clean NYS Driver’s License required.Travel is required. PHYSICAL REQUIREMENTSMust be able to lift up to 10 lbs.Must be able to drive personal vehicle to and from home visits.

Salary : $40,000 - $45,000
