Case Manager Intern

• Shadow Case Managers in office and in the field in order to gain relevant experience and knowledge to assist with case management services.
• Assist with making contact via email or phone with collateral contacts to maintain continuity of clients care with care providers, hospitals and other professionals.
• Understand and utilize agency software (Care logic & Great Plains) for billing and documentation purposes
• Enter all completed tasks into appropriate clients record.
• Assist as necessary with clerical duties in the program, such as filing, scanning and filing within client’s records.
• Follow policies and procedures established by the Guardianship Program.
• When deemed appropriate by Clinical Case Manager Supervisor, complete face-to-face visits with clients at their place of residence and complete assessment visit documentation in Care logic (without case manager supervision)
• Assist Clinical Case Manager Supervisor with record reviews, documentation, compliance, and implementation of policy and procedure.
• Respond to reasonable expectations of the Agency and/or supervisor.

**Must be able to successfully complete a MA State CORI check & CPS check.**
