Principal Job Functions:
- Client Services
- Engages in and develops trusting relationships with clients
- Respect the range of diverse cultures and the broad array of needs experienced by individuals and families.
- Assists the family in developing an individualized support network.
- Provides services to clients in their home or community setting.
- Maintains active and regular contact with clients, including at least one face-to-face contact per month.
- Engages clients in developing their goals.
- Provides parent educational/skill building and/or emotional support to families.
- Supports clients with information, tools, and resources they need to develop new competencies and become their own advocates.
- Works cooperatively with program partners to secure housing and related support services for clients.
- Connects clients to needed mainstream and community resources.
- Provides client transportation, using personal vehicle.
- Provides regular updates to referral sources or other involved parties.
- Participates actively in supervision to better meet the needs of clients.
