What are the responsibilities and job description for the Licensed Practical Nurse (LPN) position at Family Service Association - Fall River?

We are currently seeking 1 part time (30 hours per week) LPN at our Bridgeview Adult Day Health program in Fall River, MA. The hours are Monday through Friday 8:00 a.m.- 2:00 p.m.
The ideal candidate will assist the RN as liaison between participant's primary care physician and will monitor participant's health status and care plan. Some of the responsibilities are to complete medical assessment forms, provide case management services as designated by RN; review and supervise participant's personal care services; attend monthly supervisory meetings with RN; writing monthly nursing progress notes in each participant's record; and assist the Program Manager with the implementation of policies and procedures.
Education and Requirements:
- Must be currently licensed in the Commonwealth of Massachusetts and in good standing.
- C.P.R. Certified
- Two years experience working with aging and disabled population strongly preferred.
- An ability to work with others as part of a team and communicate effectively.
Bi-lingual English/Spanish and English/Portuguese speaking applicants are encouraged to apply.
