What are the responsibilities and job description for the Housing Navigator/Case Manager position at Family Promise of Sussex County?
Family Promise of Sussex County is looking to hire full and part-time case manager -housing navigators to assist clients in achieving housing stabilization. The staff member will work with households experiencing a housing crisis in Sussex, Warren, and Hunterdon Counties, and create long-term housing stability plans. The worker will provide advocacy, support, access to resources, and referrals through one-on-one case management and group sessions.
The staff member is responsible for providing case management and housing search and placement services. Documentation is to be maintained for all program participants in the online data management system.
Responsibilities include, but are not limited to:
- Complete, intake, assessment, referral packets, and other paperwork as needed to support the client in accessing housing resources
- Collaborate closely with other staff and outside agencies in meeting client needs
- Establishes rapport with clients to obtain and document the essential data as required by program funders and agency
- Engage the services of collaborative agencies, realtors, and landlords to meet client household needs
- Assist clients in scheduling and maintain appointments for medical visits, housing search, applications for benefits and other resources as needed
- Complete Prioritization Tool and enter client data to waitlist for availability of units
- Attend Continuum of Care Case Conference meetings and assist with the facilitation of case review
- Enforce agency policy and procedural rules and regulations of the program to ensure safety for both client and employees
- Remain current on knowledge of area resources relevant to agency and client needs
- Data input and maintenance utilizing the Homeless Management Information System (HMIS) database and other spreadsheets as needed
- Utilize appropriate crisis intervention techniques for client stabilization
- Accurately assess risk to self, others or property and the presence of a grave disability
- Develop and document appropriate safety plan
- Provide ongoing advocacy, support, and services to maintain stabilization of households of clients
- Create monthly, quarterly, and annual reports for program outcomes, annual performance reports, program monitoring, and others as required by administration and grant funder
- Complete all required progress notes according to agency comprehensive guidelines, policies and procedures regarding format, content and timeliness.
Requirements
- Minimum Bachelor Level with 3 year experience in case management, housing navigation, or other areas related to housing stabilization.
- Must have a valid driver's license in good standing

Pay: From $23.00 per hour
