Schedule: Monday- Friday | Philadelphia, PA

About us:

Family Promise of Philadelphia (FPP), formerly Philadelphia Interfaith Hospitality Network (PIHN), works to prevent and alleviate family poverty and homelessness. We aim to provide a holistic array of services utilizing a trauma informed and “housing first” approach to ensure families can maintain in their current residence and have housing stability. Our goal is to resolve and prevent problems preventing household stability and self-sufficiency. Through our core programing, families receive a wide variety of services.

Position Objectives:

The role of the Residential Case Manager is instrumental in meeting homelessness prevention with trauma-informed care. The Residential Case Manager is an organized, compassionate, motivated, and dedicated social worker that will assist participants in our Promise to Shelter Program in creating and meeting goals, and gaining support with housing sustainability. These functions are directed at specific outcomes for helping individuals increase independence, develop life skills, and successfully reintegrate into the community with sustained housing.

This part-time position will report directly to and be supervised by the Director of Family Services.

Key Responsibilities:

· Support a safe, structured, and therapeutic residential environment for all participants.

· Orient new participants into the Promise to Shelter Program; Provide physical and therapeutic support on Move-In Day.

· Meet regularly (once a week minimum) with Promise to Shelter Program participants (either in static shelter, rotating shelter, or transitional housing). Provide individual counseling, life coaching, and/or supportive services to participants.

· Collaborate with participant to develop successful strategies for identifying, achieving, and maintaining goals in four key areas: Housing, Financial, Personal, and Self Care.

· Identify a person's strengths, needs, and barriers in accomplishing the identified goal.

· Collaborate with the team to develop, implement, update and review individual and family service plans that are family-oriented, strengths-based, and person-centered.

· Maintain accurate and current case documentation in the participant’s record. Report any behavior, health issues, or safety concerns regarding the participant or family member, and document in the participant’s record.

· Uphold guest handbook policy for compliance with participants, and adhere to guest rights, discipline action procedures, and grievance procedures.

· Demonstrate professional boundaries and appropriate self-disclosure when interacting with participants.

Additional Responsibilities:

· Assist clients in building independent living and community integration skills

· Support individuals to find and use resources and supports in their community.

· Teach residents in some of the activities of daily living including but not limited to personal hygiene, meal preparation, housekeeping, etc.

· Participate in the implementation of new programs for participants, crisis intervention plans and safety plans, and discharge planning.

Education Requirements:

· Bachelor's Degree Social Work (BSW), or related discipline, such as psychology, human services, behavioral health, sociology, etc. - Required

· Master’s Degree Social work, (MSW), or related discipline such as psychology, human services, behavioral health, sociology, etc. - Preferred

Experience Qualifications:

· At least two (2) years experience in social work and/or direct service, related to residential, housing, shelter, and/or family services - Preferred

· At least three to five (3-5) years if degree is in a another discipline, related to residential, housing, shelter, and/or family services - Required

Skills and Abilities:

· Experienced case management skills, including trauma-informed care and a strength-based approach.

· Must possess a positive attitude, modeling this for families we serve.

· Demonstrates openness, patience, and unprejudiced contact with socially disadvantaged people

· Ability to act independently, autonomously and exercise appropriate clinical judgment.

· Ability to identify and resolve problems in a timely manner.

· Excellent communication, time management and organizational skills.

· Ability to work effectively with all departments and all levels of professionals.

· Excellent customer service orientation and strong interpersonal skills.

· Computer skills that include data input into multiple systems such as MS office 365 and Google Workspace

· Comfortable working in environment that promotes Inclusion, Access, Diversity, and Equity.

· Able to lift up to 25 lbs.
