POSITION SUMMARY:


Uphold the health center mission by assuring that all patients receive health care that is competent, caring, cost-effective, and meets with a high level of satisfaction: supports the fiscal stability of the organization. 




The Primary Care Site Director serves as the primary clinical leader for an assigned health center site within the FQHC network. This role combines direct patient care responsibilities with site-level clinical leadership, quality oversight, provider mentorship, workflow optimization, and interdisciplinary team collaboration.




PC Site Director ensures delivery of high-quality, patient-centered, culturally responsive, and evidence-based primary care services consistent with FQHC, HRSA, PCMH, value-based care, and regulatory requirements. The position partners closely with site operations leadership to improve patient access, clinical outcomes, patient experience, provider engagement, and care team performance.




The Lead NP maintains an active patient panel while serving as the clinical resource and operational liaison for providers and clinical staff at the site.


 


ESSENTIAL DUTIES & RESPONSIBILITIES:


Clinical Care



  • Provide comprehensive primary care services across the lifespan in accordance with scope of practice and organizational protocols. 

  • Diagnose and treat acute and chronic medical conditions. 

  • Perform preventive care, screenings, and health maintenance services. 

  • Prescribe medications and treatments in accordance with state and federal regulations. 

  • Manage patient panels with focus on continuity, quality outcomes, and access. 

  • Coordinate care with behavioral health, dental, specialty care, pharmacy, care management, and community-based services. 

  • Ensure accurate, timely, and compliant clinical documentation in the electronic health record (EHR). 




Site Clinical Leadership



  • Supervise site Nurse Practitioners, Registered Nurses and Practice Manager

  • Serve as the clinical lead and day-to-day provider resource for the assigned primary care site. 

  • Promote evidence-based practice standards and clinical consistency across providers. 

  • Support implementation of organizational clinical initiatives, workflows, protocols, and quality improvement efforts. 

  • Collaborate with site leadership to address clinical workflow challenges, staffing needs, and patient care issues. 

  • Facilitate regular provider huddles, case reviews, and interdisciplinary collaboration. 

  • Escalate clinical concerns, patient safety issues, and operational barriers to senior clinical leadership. 

  • Support onboarding and orientation of new providers and clinical staff. 

  • Participate in provider mentoring, coaching, and peer support activities. 




Quality & Population Health



  • Support achievement of organizational quality metrics including: 

    • HEDIS measures 

    • UDS measures 

    • Preventive care benchmarks 

    • Chronic disease management outcomes 

    • Value-based care performance indicators 


  • Review site-level clinical quality data and support improvement initiatives. 

  • Promote patient retention, care gap closure, and preventive screening compliance. 

  • Participate in clinical audits, peer review, and quality assurance activities. 

  • Assist with implementation of care management and population health strategies. 




Operational Collaboration



  • Partner with Access Manager and Practice Manager to improve: 

    • Patient flow 

    • Scheduling optimization 

    • Access management 

    • Clinical staffing coordination 

    • Same-day visit utilization 

    • Team communication 


  • Assist with provider schedule management and coverage coordination. 

  • Support reduction of avoidable emergency department utilization and hospital readmissions. 

  • Participate in organizational committees, leadership meetings, and strategic initiatives as assigned. 




Compliance & Risk Management



  • Ensure compliance with: 

    • HRSA requirements 

    • FQHC regulations 

    • OSHA standards 

    • HIPAA regulations 

    • Infection prevention protocols 

    • Clinical quality standards 


  • Report adverse events, safety concerns, and compliance issues appropriately.




EXPERIENCE AND REQUIREMENTS:
Candidates should possess a diverse range of skills and experience including but not limited to:


Required Education and Experience


Education



  • Master’s or Doctoral degree from an accredited Nurse Practitioner program. 


Licensure & Certification



  • Active and unrestricted Nurse Practitioner license in the Commonwealth of Pennsylvania. 

  • National board certification as a Nurse Practitioner. 


Experience



  • Minimum of 3–5 years of primary care clinical experience required. 

  • Experience in community health, FQHCs, underserved populations, or safety-net healthcare strongly preferred. 

  • Prior leadership, precepting, charge, supervisory, or provider mentoring experience required. 

  • Experience with value-based care and quality metrics preferred.


 


Required Skills and Abilities 



  • Strong clinical assessment and diagnostic skills. 

  • Knowledge of FQHC operations, HRSA requirements, and population health models. 

  • Ability to lead interdisciplinary clinical teams collaboratively. 

  • Strong communication, coaching, and conflict resolution skills. 

  • Experience using electronic health records and clinical reporting tools. 

  • Commitment to health equity, culturally responsive care, and trauma-informed practice. 

  • Ability to balance clinical productivity with leadership responsibilities.




Other Qualifications



  • Ability to pass required background screenings and credentialing verifications. 

  • Required Immunizations: Hepatitis B, TB/PPD Test.

  • Optional Immunizations: COVID and Flu Vaccination. 

  • Current valid CPR Certification is required.

  • Maintain current valid applicable position credentials.

  • Maintain reliable transportation to travel throughout the network and/or participate in events outside the network as needed. 

  • Must be willing to work on-site.




Lifting Requirements


LIGHT – Exerting up to 20 pounds of force frequently, and/or negligible amount of force constantly to move objects. The use of arm and/or leg control requires force greater than sedentary, but worker still sits majority of time.
