Responsibilities
- Supervise and coordinate daily activities of patient care assistants and care coordinators in the field, ensuring adherence to clinical protocols and organizational policies
- Collaborate with multidisciplinary teams including nursing, social work, behavioral health, and discharge planning to develop comprehensive care plans tailored to individual patient needs
- Monitor patient progress through EMR (Electronic Medical Record) systems and EHR (Electronic Health Records), documenting all interactions accurately and timely
- Conduct regular site visits to hospitals, hospice facilities, and other healthcare environments such as ICU, PICU (Pediatric Intensive Care Unit), Level I/II trauma centers, ensuring quality standards are maintained
- Facilitate intake assessments, utilization review, utilization management, and case management activities to optimize patient outcomes and resource utilization
- Provide training and mentorship to field staff on medical terminology, HIPAA compliance, CPT coding, ICD-9/10 coding, discharge procedures, and emergency protocols
- Lead efforts in emergency response coordination during critical incidents or trauma situations, ensuring swift communication with first responders when necessary
Skills
- Extensive experience in critical care settings such as ICU, emergency medicine, or trauma centers with a strong understanding of acute care procedures
- Proven hospital experience with familiarity in hospital medicine workflows and managed care processes
- Strong background in nursing or social work with knowledge of behavioral health and hospice care practices
- Expertise in medical coding including CPT (Current Procedural Terminology), ICD-9/10 coding systems for accurate billing and documentation
- Proficiency with EMR/EHR systems ensuring compliance with HIPAA regulations and maintaining patient confidentiality
- Excellent case management skills with experience in discharge planning, intake processes, utilization review, and utilization management
- Ability to lead a team effectively while managing multiple priorities in fast-paced healthcare environments such as Level I/II trauma centers or pediatric units
- Strong communication skills for liaising between healthcare providers, patients, families, and external agencies
