Patient Relations Outreach Representative

Position Overview

The Patient Relations Outreach Representative is responsible for growing Family Medical Group's patient base by building strong relationships within the community throughout the Kendall, Homestead, and Coral Gables markets.

This individual will educate Medicare beneficiaries about Family Medical Group's services, develop community partnerships, support center activations, and work closely with insurance agents, brokers, community organizations, and local businesses to generate new patient opportunities while ensuring an exceptional patient experience.

Key Responsibilities

• Enroll eligible Medicare patients with Family Medical Group as their Primary Care Provider (PCP).
• Develop and execute community outreach initiatives throughout the Kendall, Homestead, and Coral Gables service areas.
• Build relationships with senior communities, churches, pharmacies, assisted living facilities, local businesses, community organizations, and referral partners.
• Generate qualified Medicare leads through community events, presentations, health fairs, patient referrals, networking, tele-outreach, and grassroots marketing.
• Collaborate with the Marketing team to execute center activations, tabletop events, educational seminars, and community partnerships.
• Meet or exceed monthly new patient enrollment goals.
• Educate prospective patients on Medicare options and coordinate with licensed insurance agents and brokers when appropriate.
• Explain Family Medical Group's services, providers, wellness programs, transportation resources, and patient benefits.
• Coordinate new patient appointments, center tours, and transportation when needed.
• Partner closely with Center Administrators, Care Coordinators, and Case Management to ensure a seamless onboarding experience.
• Maintain ongoing relationships with patients to support retention and patient satisfaction.
• Track outreach activities, referrals, partnerships, and enrollment results.
• Represent Family Medical Group professionally within the community while serving as a brand ambassador.

Qualifications

Education

• High School Diploma or equivalent required.
• Associate's or Bachelor's degree preferred.

Experience

• Healthcare marketing, patient outreach, community relations, business development, or sales experience preferred.
• Experience working with Medicare Advantage populations is highly preferred.
• Experience building community partnerships is a plus.

Skills

• Excellent communication and interpersonal skills.
• Strong relationship-building abilities.
• Self-motivated and goal-oriented.
• Comfortable presenting to groups and attending community events.
• Basic computer and CRM skills.
• Reliable transportation for community outreach.

Language

• English required.
• Spanish preferred.
• Creole is a plus.

Pay: $24.00 - $29.00 per hour
