What are the responsibilities and job description for the RN Case Manager- Hospice position at Family Hospice of Belleville Area?
PURPOSE: To assist the Hospice Interdisciplinary Team in direct patient care by providing compassionate nursing services and support to the patient, family and the community dealing with a terminal illness.
ORGANIZATIONAL RELATIONSHIP: This position reports to the Patient Care Coordinator.
FUNCTIONS AND DUTIES OF THE JOB: (This list is not exhaustive and may be supplemented as necessary.)
- Provide patient nursing care.
- Evaluate and make recommendations of hospice candidates-patients for admission-discharge to the program.
- Assume a caseload of patients for primary nursing.
- Complete the nursing process of assessment, development of a plan, implementation and evaluation from admission to discharge of assigned patients.
- Obtain physicians orders, writes and administers the plan of care for each assigned patient.
- Develops a plan of care for assigned certified nurses aides and schedules supervisory visits per agency policy.
- Instruct patients and families on administering medications, disease process, symptom control measures and other treatments as prescribed by the patient’s physician and care plan.
- Apply appropriate cost containment policy when administering the care plan.
- Participate in Interdisciplinary Group meetings and nursing department meetings to direct nursing plan for patients.
- Participate in a 24 hour on-call rotation.
- Provide support for patient, family and the community dealing with a terminal illness.
o Provide emotional support to both patient and family members specific to each individual patient.
- Document patient care activities consistent with agency requirements.
o Complete appropriate records and report changes in patient’s condition in accordance with agency policy and procedure.
- Assist with development and presentation of educational programs, both formal and informal.
o Assist with orientation and skill training for staff.
o Assist with orientation and training for volunteers.
o Assist with educational training for outside third parties.
o Provide care and health education to each patient and their family consistent with the individualized care plan.
o Provide on-going education to the caregivers providing care to the hospice patient.

§ Receive continuing education training.
§ Review of patient records to ensure quality and timeliness of documentation in quality assurance program as assigned.
§ Assists with special projects on an as-needed basis.
§ Complies with the organization’s policies and procedures related to protection of health information and patient privacy.
- Skill in use of various computer applications and Microsoft Office products including: word, power-point, spreadsheet and web site software.
KNOWLEDGE/SKILLS/ABILITIES:
Education successfully completed formal educational training in nursing, Bachelor degree preferred.
Experience one year nursing experience which includes experience in geriatric, long term care or hospice environment.
Licensure current from State of Illinois as registered nurse.
Certification Hospice and Palliative Care Certification preferred.
Salary : $30 - $38
