Family Home Health is seeking an Intake Coordinator who is ready to make a difference in the lives of home health patients. As an Intake Coordinator you'll be part of a collaborative team whose energy, communication and dedication are valued and rewarded. The ideal candidate possesses a valid Registered Nurse License; strong Home Care Home Base system skills; be multi-task and deadline oriented; at least 2 years' experience in home health care . This position is vital to interacting with referral sources and directing all patient referrals, intake, and scheduling operations. Will work in conjunction with administrative staff to ensure best intake and scheduling practices for our visiting staff and patient care. Will generate weekly reports for ongoing evaluation with Executive Team. This position is held by an essential employee and is vital to maintaining the quality of care given to our residents and patients during the coronavirus / COVID-19 pandemic and beyond.
