We are looking for a Certified Family Nurse Practitioner to join our team to provide exceptional medical services to the patients of Family Health Services. This position is a full-time day shift position working Monday through Friday in an out patient setting at our Medical Center in Downtown Sandusky.

Key Responsibilities
- Completes patient history and physical and document all patient care provided.
- Formulates and directs plan of care for the patients and refers to other physicians, specialists as needed.
- Embraces the integrated care model and provides holistic quality care while collaborating with the whole team.
- Prescribes medication in accordance with Ohio Revised Code and Ohio Administrative Code and Formulary provided by the Advanced Practice Nursing laws.
- Complies with State Advanced Practice Nursing Guidelines/Law on Practicing Medicine.
- Responds to medical emergencies and acts as a leader in these situations.
- Directs patient education for health education, maintenance, emotional and physical needs.
- Participates and provides care as according to the chronic disease model.
- Establishes and maintains professional working relationship with other members of the health care team.
- Serves as a role model for clinical staff.

Required Education And Experience
- Must hold a diploma from an accredited School of Advanced Practice Nursing.
- Licensed to practice as a Nurse Practitioner in the State of Ohio.
- Current Nurse Practitioner Certification.
- Nursing experience in ambulatory care setting, community health setting is ideal.
- 3 years’ experience in practice as a nurse practitioner.
