Location:Marathon (in-person, clinic environment)

Required Qualifications:

· Graduate from an AMA/NCCPA/AAPA accredited Physician Assistant Program, or, an Accredited Nurse Practitioner Program.

· Current licensure with New York State Department of Education. Maintain appropriate AAPA/ANA certification.

· Must work and communicate effectively and collaboratively with: patients, families, Family Health Network staff, hospital personnel, EMS, police, fire department, community schools and other community groups and organizations.

Primary Function: The Physician Assistant (P.A.) / Nurse Practitioner (N.P.) provides primary diagnostic and therapeutic care to patients in multiple settings.

General Summary of Duties:

Provide evidence-based, empathetic, and patient-centered primary care to patients. Provision of care within accordance with Bylaws, Rules, Regulations, and policies/procedures of the health centers; including all applicable state and federal laws, rules, regulations, codes, directive and memoranda; conditions of participation in Medicare, Medicaid and other financial reimbursement or grant providers; the directions of the administrator, senior physician or staff physicians. Shall provide such care in accordance with the current ethical and professional standards of the AMA, AAPA, AAFP and AAP.

  • Though care will primarily be provided in the clinic setting, patient needs may require care setting to be hospital or home based at times.
  • Continuously exhibit knowledge, skills, and performance relating to, but not limited to, the specific qualifications / knowledge and skill requirements indicated in the attached addendum.
  • Evaluate the health of the individual including:

- Carry out a thorough history and physical examination.

- Request and/or perform certain diagnostic tests and interprets results.

- Distinguish normal from abnormal in medical history, physical examination, and diagnostic tests sufficiently to make a general health assessment, taking into consideration ever changing psychosocial and family aspects of care.

- Diagnose

- Maintain contact and rapport with the family.

- Manage common acute and chronic illnesses.

  • Refer complex and high priority cases to the physician or other health specialists as appropriate.
  • Promote and work as a member of the health care team in professional collaboration with the physician.
  • Plan patients’ health care maintenance with the clinical team.
  • Participate in assigning tasks to other members of the health team in line with their roles.
  • Participate in community health care with the Family Health Network team and with members of the community.
  • Efficiently, effectively and timely manage the patient’s course of treatment including:

- Order appropriate laboratory, radiology and other diagnostic services

- Assisting in the patients’ potential hospital admission and discharge process

· Attend local health related meetings to promote various center services, and/or to represent the center to receive suggestions or concerns. Suggestions or concerns outside the responsibility or authority of the PA/NP to respond to should be immediately forwarded to the administrator.

· Responsible for satisfying personal continuing education requirements per state licensure requirements. Attendance during normally scheduled work hours must be approved in advance by the administrator.

· Assist the Chief Medical Officer (CMO) and Family Health Network in identifying changing community health patterns by:

- Maintain contact and rapport with community leaders and organizers.

- Support the evaluation of the health status of the population by outside agencies with the approval of the CMO.

- Maintain communications with other medical and health providers within the community.

· Maintain BLS certification.

· Provide employee examinations as approved by the administrator (except in emergencies).

· Provide examination and/or treatment of employees, visitors and patients injured on center property.

· Continuously maintain current knowledge and skill levels to:

- Perform complete newborn, infant, pediatric, adolescent, teen, adult, and geriatric physical examinations and health assessments.

· Make diagnostic and therapeutic clinical management decisions relating to:

- Behavioral/Psychiatric crisis intervention

- URI’s

- Pneumonia

- UTI’s

- GI diseases

- Vision Screening

- Corneal abrasions/trauma

- Hypertension

- COPD

- Diabetes

- Peptic disease

- Pelvic exam

- Pap smears

- Birth control education: Oral contraceptives, IUD’s diaphragms, Norplant and Depo-Provera

- Prenatal education/care

- Emergency deliveries

- Perform basic lab tests: Urinalysis, hematocrit, gram stains, wet drop preparation

- SED rates and cultures

- Arthritis

- “Senility”

- Depression

- Parkinsonism

- Other geriatric medical concerns

- Counseling-Dietary, behavioral and family planning

- Prescribing non-controlled substances

- Minor surgical procedures: Incision and drainage, small cyst excision, electric cauterization, joint effusion, laceration or puncture suturing.

- Burns

- Shock Management

- CVA’s

- Multiple trauma

- Acute asthmatic attacks

- Seizures

- Perform and provide initial EKG interpretations

- Allergy injections

- Tympanogram and other functions as may be requested by Physicians.

  • PA/NP may be asked to participate in quality improvement activities, which include but are not limited to, performing PDSA, administering Patient Satisfactions Surveys and following up on high risk patients based on population reports for patients with poor clinical outcomes or with lapses in care.
  • PA/NP is part of the patient care team. The care team is comprised of the patient, care coordinator, nurse, and provider. As part of the patient care team, the PA/NP will engage patients in their own health care. This will include supporting patients and their families in self-management and positive behavior change. This will require good communication which is both open and supportive.

· This position requires compliance with Health Center’s written standards, including its Standards of Conduct and policies and procedures pertaining to all operational aspects, furthermore this position is required to report any suspected instances of noncompliance.

· This position will function as a contributing member of the FHN Dramatic Performance Improvement Model of Care to ensure that FHN’s Team-Based Care is the most sustainable, efficient method for providing high-quality, satisfying, and equitable health care to our patient population.

· This position will function as a contributing member of the emergency preparedness team under the incident command system.

· Maintain strictest confidentiality at all times.

Pay: $110,000.00 - $140,000.00 per year
