This position is an integral part of the behavioral health / primary care team providing quality, comprehensive care, by providing accurate, effective coordination between the patient and mental health or primary care provider. Assists with other patient care coordination activities as directed by the supervisor.
Facilitates the patient to communicate with the behavioral health providers, primary care providers and other FHC staff during the patient visit. Works as back-up for the medical support staff team. Must possess a basic level of written and verbal communication skills, computational and computer skills, and mathematical knowledge typically acquired through completion of a high school program.
