Key Responsibilities
- Conduct initial and ongoing assessments of clients' health and support service needs
- Create formalized case plans with clients, identifying problems and needs, and developing written care-plans with short and long-term goals
- Provide therapeutic interventions to clients, including accompanying them to appointments and social services
- Complete all required documentation accurately, in a timely manner, and thoroughly in accordance with department standards
Requirements
- 3 years of work experience providing human services to high-risk, medically underserved, disabled populations, and/or relevant community health populations
- Arabic and Chaldean fluency required
- Bachelor's degree in Social Science field, Public Health, Healthcare Administration, or closely related field required; Master's degree in relevant field preferred
Salary : $29 - $35
