Schedule:

Education and Experience

Master of Science in Nursing (MSN) from an accredited institution required

Current Advanced Practice Registered Nurse (APRN) licensure in the state of Kentucky

Active Family Nurse Practitioner (FNP) certification

Licensure and Certification

Certified Family Nurse Practitioner (FNP) / APRN-CNP required

CPR and/or BLS Certification required

Drivers License

Must remain in good standing with the Kentucky Board of Nursing and comply with all Kentucky state regulations

Special Skills, knowledge and abilities

Advanced clinical knowledge and decision-making skills

Proficiency in Electronic Health Records (EHR) systems and clinical documentation standards

Excellent communication, organization, and time management skills

Ability to lead meetings, set expectations, and drive accountability

Strong understanding of clinical pathways, patient follow-up standards, and care coordination

Ability to maintain confidentiality and exercise sound judgment

Summary

A Family Nurse Practitioner (FNP) has a broad range of responsibilities centered around providing primary and specialized care to individuals and families across all age groups. FNPs are responsible for conducting thorough assessments, including taking health histories

and performing physical examinations, to evaluate patients’ health needs. They diagnose and treat acute and chronic conditions, managing everything from minor illnesses to more

complex medical issues. FNPs order and interpret diagnostic tests, prescribe medications, and develop personalized treatment plans that may involve lifestyle modifications, counseling, and

preventive care. They emphasize health promotion and disease prevention, providing education on nutrition, exercise, mental health, and other wellness topics. Additionally, FNPs

coordinate patient care, collaborating with physicians, specialists, and other healthcare professionals to ensure comprehensive treatment. They may also provide ongoing

management for chronic conditions, such as diabetes or hypertension, and support patients and families in making informed healthcare decisions. In some settings, FNPs also have the

responsibility of supervising or mentoring other healthcare staff and staying updated on the latest clinical practices through continuing education.

Responsibilities

Providing health promotion, prevention, and treatment to patients of all ages.

Support timely access to care by maintaining an expected volume of approximately 20 patients per full clinic day, ensuring continuity of care for the community served.

Diagnose and treat acute and chronic conditions, including but not limited to diabetes, high blood pressure, infections, injuries, and concussions.

Prescribe medications as needed to support patient health and well-being.

Order, interpret, and evaluate diagnostic tests.

Gather comprehensive health histories, perform physical examinations, and conduct diagnostic procedures.

Develop, implement, and modify individualized treatment plans based on patients needs.

Dictate and document all patient interactions accurately and in a timely manner.

Notify the managing physician about any unusual or unexpected events or problems.

Communicate effectively with patients, families, and diverse populations, ensuring clarity and understanding.

Collaborate with medical team members to coordinate and enhance patient care.

Ensure to update and provider HR with all licensure in a timely manner.

Promote positive relationships between FHCA, schools, and community partners

Other roles/duties will be assigned as necessary to assist FHCA in the attainment of the goals set forth and the enhancement of a positive, respectful family practice for all patient and employees.
