Family Guidance of Texas (FGOTX) is seeking compassionate and motivated Case Managers/Case Workers to join our behavioral health team. The ideal candidate will provide support, coordination of care, community resources, and client advocacy for individuals receiving mental health services.
Responsibilities:
- Coordinate client care and referrals
- Maintain accurate documentation and case notes
- Assist with treatment goals and service planning
- Communicate with providers, families, and community resources
- Support clients with access to housing, benefits, and other services
Qualifications:
- Experience in behavioral health, social services, or case management preferred
- Strong communication and organizational skills
- Ability to work independently and as part of a team
- QMHP preferred but not required
