What are the responsibilities and job description for the Care Management Coordinator position at Family Choice of New York?
The Care Coordinator plays a crucial role in managing patient care by facilitating communication between patients, healthcare providers, and support services.
TASKS:
- Conduct a Health Risk Assessment Tool (HRAT) per Family Choice guidelines.
- Ensure Individualized Care Plan is updated and maintained per Family Choice guidelines and signed by Care Management RN/APP.
- Ensure that updates to the ICP are shared with members of the ICT including PCP per Family Choice guidelines and as stated in Model of Care.
- Outreach when a member is hospitalized and/or experiences a transition of care.
- Complete all required documentation in an accurate and timely manner per company guidelines.
- Maintain an accurate record of telephonic interactions with members, the member’s PCP or other specialist providers and family/caregiver.
- Establish trust and rapport with members /families (as appropriate) and staff in each care setting in which members receive services.
- Identify whether the member is receiving care services as ordered.
- Coordinate and facilitate members’ access to services with other health care providers as needed.
- Identify and address social determinants of health needs for members and refer to Care Management Social Work as appropriate.
- Identify potential clinical concerns and escalate to the clinical team promptly so that further direction can be given.
- Assist with scheduling in-home visits for other care management staff.
- Ensure HIPAA protocols and documentation have been completed as per company guidelines.
- Assist with arranging DME, transportation, and other services as requested by the Interdisciplinary Care Team.
- Participate in Interdisciplinary Care Team (ICT) Conferences.
- Other duties as assigned.
EDUCATION: Associate’s or Bachelor’s degree preferred.
EXPERIENCE/ABILITIES:
- Minimum two (2) years working in a call center and/or an administrative support role preferably in a health care setting.
- Excellent verbal and written communication skills.
- Ability to articulate and deliver accurate and clear communications via written and/or oral communication.
- Demonstrated pleasant telephone manner.
- Versatile and well organized.
- Requires strong problem-solving skills.
- Strong time management skills.
- Must be able to build and maintain collaborative, cooperative, and productive relationships with all internal and external customers including staff, members and their families, nursing facility staff, physicians, the health plan, and any other stake holders in the community.
- Strong keying/data entry skills necessary.
- Computer literate with proficiency in a wide variety of applications.
- Must be flexible and able to work in a team environment.
REQUIREMENTS:
- CPR certification
- Valid NYS Driver’s License and active automobile insurance to meet company requirements
- Transportation
