POSITION SPECIFIC DUTIES & RESPONSIBILITIES:    

  • Manages the day-to-day unit operations and workflows including admits, transfers, and discharges  
  • Oversees/manages chair and bed occupancy to ensure maximum utilization and efficiency
  • Ensures all units are properly staffed according to acuity.  Ensures each unit is therapeutic, safe and conducive to meeting the needs of the clients served. 
  • Ensures the unit milieu is consistently followed
  • Ensures services provided and client record documentation maintains compliance with standards/criteria/regulations of accreditation and purchases of services, i.e, ODMHSAS, COA, OHCA etc.
  • Participates in interdisciplinary case conference to link community resources with client treatment needs in the formulation of admission, discharge, and referral depositions
  • Ensures timely movement on front-end and back-end processes of the admission, transfer and discharge system to meet outcome targets
  • Works as part of a cohesive unit team that ensures all roles are being performed and covers various unit staff roles as needed
  • Responds to all unit codes by assisting with restraints and seclusions by utilizing SafeClinch and Therapeutic Options
  • Provides bed-finding for URC clients needing a higher level of care
  • Assists unit clerk in assuring all billing associated w/ admits, transfers, and discharges are documented properly
  • Ensures/assists all eligible clients in applying for Medicaid benefits
  • Handle all incoming referral calls from ERs, mental health facilities, etc.
  • Assists with transfer and discharge processes as needed
  • Complete discharge paperwork with clients discharging to self from URC
  • Obtain collateral information as needed for prescribers/triage to help them determine appropriate level of care for clients on URC
  • Complete follow up calls to discharged patients to ensure linkage to services. 
  • Works with discharge planners in management of client visitation

Requirements

  • Bachelor’s Degree in the social sciences field is required, with consideration given for degrees in Criminal Justice or Education.
  • Case Management Certification level II or obtain by taking the first available Case Management level II Certification course provided by ODMHSAS after hire.
  • Must have a strong commitment to the right and ability of each person served to live in normal community residences; and have access to helpful, adequate, competent, and continuous supports and services.
  • Skills and competence to establish supportive trusting relationships with persons with mental illnesses and/or substance abuse issues and respect for client rights and personal preferences in treatment are essential. 

 

CCBHC Model of Care

Family & Children’s Services works to heal hurting and abused children, strengthen families, and provide hope and a path to recovery for those battling mental illness and addiction. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC).  This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.

Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:     

  1. Increased access to care and crisis services
  2. Expanded traditional community mental health and substance use services.
  3. Added Care Coordination and physical health screening for mental health clients
  4. Greater access to Social Services for clients’ economic and social needs
  5. Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.
