Registered Nurse

Finding a nurse for a Certified Community Behavioral Health Clinic (CCBHC) requires a unique blend of clinical expertise and understanding of the mental health system. These roles are not just about vitals; they are about integrated care—treating the "whole person" by bridging the gap between physical and mental health.

The CCBHC Nurse is a vital member of a multidisciplinary team dedicated to providing integrated, community-based care. You will work alongside therapists, psychiatric nurse practitioners, doctors and peers to provide health screenings, medication management, and chronic disease education to individuals mental illness, substance use disorders and co-occurring physical health conditions.

Work Environment:

This is a professional, full-time (35 hours/week) position primarily in office at our Cortland clinic with required travel to Binghamton and Oneida satellite clinics.

Responsibilities and Typical Work Activities:

· Conducting comprehensive assessments of patients’ health needs, developing individualized treatment plans, and monitoring progress.

· Ensuring individuals have access to primary care services.

· Ensuring ongoing periodic laboratory testing and physical measurement of health status indicators and changes in the status of chronic health conditions.

· Ensuring outpatient primary care screening and monitoring of key health indicators and health risk are received timely

· Coordinating care with primary care and specialty health providers including tracking attendance at needed physical health care appointments.

· Promoting a healthy behavior lifestyle including nutritional advice and smoking cessation.

· Administering Intramuscular Injections as ordered by Doctor or Psychiatric Nurse Practitioner

· Educating patients on prescribed medications; monitoring for potential medication interactions and side effects as requested by their prescriber

· Responding to immediate health crises, providing de-escalation techniques, and coordinating emergency services if needed.

· Managing patient care coordination, including communication with other healthcare providers, community support services, and family members.

· Providing psychoeducation on health conditions, coping mechanisms, and recovery support strategies.

· Accurately documenting patient encounters, assessments, treatment plans, and medication administration in electronic health records.

· Understanding and addressing the impact of trauma on patients' health care administration is critical in a CCBHC setting.

· May assume additional responsibilities as required.

· Applicants should demonstrate an awareness and understanding of LGBTQ issues, and practice gender-affirming interactions with others.

Required Qualifications:

· Current licensure in NY as a Registered Nurse

· Active CPR certification

· Phlebotomy experience required.

· Prefer experience with mental health and chemical addiction populations

· Strong organizational and multitasking abilities

· Ability to work in a fast-paced environment

· Compassionate, patient-centered and trauma informed approach

· Attention to detail and critical thinking skills

· Proficiency in electronic medical record (EMR) systems is preferred
