Role Description
This is a part-time, on-site role for a Case Manager located in Winona, MN. The Case Manager will work directly with clients to assess their needs, develop individualized care plans, and provide guidance to access services and resources. Responsibilities include coordinating with other professionals and agencies, conducting home visits, maintaining client documentation, and advocating for client needs to achieve their goals. The role also requires tracking client progress and ensuring compliance with organizational policies and applicable regulations.
Qualifications
- Client Assessment, Case Planning, and Resource Coordination skills
- Strong interpersonal and communication abilities, including advocacy and counseling
- Organizational and documentation skills with attention to detail
- Proficiency in time management to handle a varied caseload effectively
- Ability to collaborate with multidisciplinary teams and external agencies
- Familiarity with child welfare, mental health resources, and community support programs is a plus
- Bachelor's degree in Social Work, Psychology, or a related field is preferred
- Relevant certifications or licenses (e.g., Social Work certification) are beneficial
