Responsibilities
- Provide case management to veterans and their families/caregivers in-office and in the community using motivational interviewing
- Conduct screenings, advocate for clients, and connect them with appropriate services and resources.
- Collaborate with veteran service providers, advocates, and navigators to streamline care coordination, enhance service delivery, and support organizational initiatives addressing care-access barriers.
- Engage in community outreach across Long Island to link veterans and families with FCA services.
- Manage a caseload by developing, implementing, and monitoring treatment plans
- Maintain timely, compliant electronic case records and complete all agency, program, and regulatory reports and documentation by required deadlines.
- Participate in interdisciplinary team meetings, case conferences, workshops, and supervision.
- Perform additional responsibilities to support the operations of the program.
- Bachelor's degree in the human service field required.
- Minimum of one year of experience in the community health care setting and with working with veterans and their families.
- Proficient computer skills with MS Office.
- Valid and clean driver's license required.
- Bilingual in Spanish is preferred.
