The Families Healing Together Program is seeking a Clinician to provides short-term counseling services to individuals ages 7-21 and their families, along with community outreach and education throughout Nassau County to increase awareness of Adverse Childhood Experiences (ACEs) and strategies to reduce risk factors. The program also facilitates a family support and education group and offers targeted case management services.

Responsibilities
- Provide individual, family and group therapy sessions, as well as any other prescribed treatment service in accordance with agency and regulatory standards.
- Maintain required productivity levels and manage an assigned caseload effectively.
- Conduct initial intake documentation and comprehensive assessments.
- Develop, implement and regularly monitor client treatment plans
- Collaborate with external service providers to support client progress and continuity of care.
- Communicate regularly with internal programs and external agencies to support client outcomes.
- Actively participate as a member of the interdisciplinary treatment team, including attending case conferences, team meetings, and in-service trainings.
- Complete and maintain accurate, up-to-date client case records in compliance with agency policies and other regulatory requirements.
- Prepare and submit all required reports, correspondence, and documentation in a timely manner, in line with agency, program, and regulatory deadlines.
- Maintain confidentiality in accordance with HIPAA.
- Provide community education on Adverse Childhood Experiences (ACEs) and other prevention based behavioral health awareness information
- Master's degree in Social Work, Mental Health Counseling, or related field.
- QHP credential preferred: LMSW, LCSW, LMFT, LMHC, LCAT; will consider candidates eligible for limited permit to be obtained within 90 days of hire.
- Minimum of 1 year experience providing individual and/or group counseling to substance use or mental health population either working in a chemical dependency or mental health program (may include internship).
