What are the responsibilities and job description for the Family Support Home Visitor position at Family Center of Washington County?
The Family Center is looking for someone to join our amazing team to provide parent education, family support, and service coordination to pregnant and parenting families with children birth to six who may be faced with multiple life stressors. You’ll work to strengthen family resiliency to improve the health and well-being of families and their children.
Responsibilities:
- Deliver home visiting services by assessing family strengths, vulnerabilities, developmental status, and risks; provide parent education, anticipatory guidance, and connect families to health and social services.
- Support families in managing stressors through coaching, culturally appropriate care plans, and interventions that build parenting skills, resiliency, and confidence in addressing life challenges.
- Collaborate with community partners to ensure child safety, coordinate case management, address service gaps, and develop integrated “One Plans” with input from other CIS providers.
- Documenting services and contributing to program evaluation.
Desired Qualifications:
- Bachelor’s degree in Social Work, Education, Public Health, or other related Human Service field.
- Experience working with families, including families at-risk, and experience with home visiting preferred.
- Knowledge of child development
