Job Summary
We are seeking a dedicated and compassionate Program/Case Manager for DD Sponsored Residential services. This role involves overseeing the delivery of high-quality care to residents with developmental disabilities, ensuring compliance with care plans, and managing day-to-day operations within a residential setting. Responsibilities
- Write, track, and amend individual service plans for up to 15 individuals receiving services.
- Work closely with Community Services Boards’ Support Coordinators, individuals, and biological families to develop the most appropriate plan for the individual; attend treatment team meetings.
- Monitor home compliance with all requirements, including documentation, human rights, and compliance with the service plan.
- Maintain client and sponsor documentation in accordance with program and state requirements.
- Assist in planning and implementing social activities for families and individuals. Attend all scheduled social activities.
- Assist in recruiting family homes and completing sponsor and client intake.
- Assist in marketing program to CSB’s, disability advocacy organizations and other individuals and organizations in the community.
- Assist in training Sponsored Home providers.
- Provide technical assistance to families regarding implementation of Individualized Services Plan.
- Arrange to be on-call for assigned caseload in case of Sponsored Home emergency.
- Handle the task and day to day responsibility of our Sponsor Residential, Community service program.
Requirements
- Work as a Program Manager for an agency in case management with a minimum of (2) years' experience working with adults or:
- Have a Human Services Degreed; Qualified Developmental Disabilities Professional (QDDP) with a minimum of 2 years' experience with the DD population
- Minimum 1 year experience in writing or maintaining service plans as well as knowledge of Licensure requirements and a demonstrated ability to meet assigned deadlines for work completion.
- Strong interpersonal skills with the ability to communicate effectively and professionally with a variety of people.
- Excellent oral and written communication skills.
- Knowledge of community resources available for individuals with cognitive impairments.
- Strong demonstrated knowledge of Human Rights and ability to effectively assist clients with self-advocacy.
- Knowledge of Trauma Informed Care principles.
- Strong attention to detail and ability to work under pressure.
- Strong computer skills including Word, Excel, and Outlook. Familiarity with Lauris & Therap EHR preferred.
- Must have a valid driver’s license, have transportation available, and be able to complete home visits for assigned case load.
- Must have and maintain a working cell phone and home internet.
- Must meet all background check and training requirements within 30 days of hire date, as well as ongoing recertification or new training requirements.
- Network of personal contacts in the Tidewater intellectual/developmental disability community a plus.
Pay: $47,000.00 - $52,000.00 per year
