What are the responsibilities and job description for the Registered Nurse Case Manager position at Family Care Home Health and Hospice?
The registered nurse plans, organizes and directs Home Health care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of he community to plan and direct services to meet the needs of individual and families within their homes and communities.
- Conducts comprehensive and ongoing assessments of both the patient and family to determine home health and hospice needs, including physical, psychosocial, and spiritual status.
- Utilizes health data to establish nursing diagnoses and sets measurable goals that incorporate palliative actions.
- Writes, initiates, and regularly revises the POC based on evolving patient status and caregiver needs.
- Executes preventive and rehabilitative procedures, administers prescribed medications, and helps patients with activities of daily living to maximize comfort.
- Instructs patients and families on self-care techniques, medication, and diet while providing ongoing health and coping counseling.
- Maintains and submits accurate notes regarding patient conditions, care administered, and pain/symptom management outcomes.
- Reports clinical changes and needs to the attending physician to secure required orders.
- Acts as Case Manager to coordinate the care plan with other health professionals and caregiver.
