Salary: $100,000 per year DOEJob Title: Intake CoordinatorSalary: $100,000 per year DOELocation: 1 day at Fountain Valley, 1 day at Garden Grove, 3 days at Tustin.Openings: 1Position Purpose: Under the supervision of the Clinical Manager, the Intake Coordinator is an essential member of the clinical team as a facilitator of clinical mental health assessments. The Intake Coordinators primary function is to facilitate the admissions process which includes risk assessments and appropriateness for services (both therapy and psychiatry).Core Duties and responsibilities, include but are not limited to:Provides clinical assessment to determine appropriateness of services using screening questionnaires and risk assessmentsCoordinates the scheduling of intakes for therapists and psychiatry providersConducts record reviews (ROI, custody paperwork, etc.)Consults with Clinical Director, Manager, or Supervisor as neededReviews internal referrals weekly, conducts outreach to referrals within 5 business days, documents attempts, and closes the loop by informing providers of outcome of referralAddresses urgent referrals within 2 business days and closes the loop by informing providers of outcome of referralCommunicates with administrative team to ensure review of patients insurance and eligibility coverage and informs the patient of copay/good faith estimateProvides individual and group therapy, as neededEnsures all appointments are properly created, scheduled, and confirmedManages waitlist and waitlist outcomesAttends meetings as applicable to the job title. (i.e. all staff meetingsBehaves professionally and works collaboratively with co-workers to increase productivityRefers therapy and psychiatry patients to outside specialists or agencies as needed, using clinical judgment and higher level of care parametersResponds to crisis situations and intervenes therapeutically as necessary; assesses for safety of patient and assesses for suicidality, homicidality, and self-harm ideationCompletes documentation within organization policy of 72 hoursAssists with quality measuresProvide clinical supervision to ASW and MSW internsPerforms other duties and projects as assigned by the Clinical Manager of Mental HealthThis job description in no way states or implies that these are the only duties to be performed by the employee.
