Job Description:
Wraparound Facilitator Overview A Wraparound Facilitator coordinates a family's care team, develops and implements individualized Wraparound plans, and ensures the process is youth-guided and family driven by helping to identify and connect with strengths, services, and natural supports in the community. They act as a coach and guide, facilitating communication, managing meetings, and monitoring progress toward the family's vision for their future.
Key Roles and Responsibilities
- Team Building: Facilitates the development of a collaborative "child and family team" by identifying and bringing together formal service providers, community members, and natural supports important to the youth and family.
- Family Engagement: Partners closely with the family to understand their needs, strengths, culture, and dreams, ensuring the entire process is centered on their goals and choices.
- Plan Development: Leads the team in developing a strengths-based, highly individualized plan of care that addresses the needs of the child or youth and family.
- Implementation and Monitoring: Oversees the implementation of the plan, coordinates services, and regularly monitors the team's progress and the effectiveness of the plan.
- Crisis Management: Helps the family and team develop strategies for managing and responding to crises and safety issues.
- Facilitating Meetings: Manages team meetings to ensure everyone has an opportunity to participate, share and contribute to the team's decision-making.
- Connecting to Resources: Links families to formal services and informal, community-based supports. ideas,
- Transition Planning: Supports the family in developing a plan to manage their own services and supports after the formal Wraparound process concludes.

Qualifications:
- Bachelor's degree in a human services field * Two or more years of relevant post-college experience working with children and families
- Must obtain CANS certification and Wraparound facilitation training
- Other qualifications include knowledge of community resources, inter-agency coordination, strong communication skills, and a commitment to the strengths-based principles of the wraparound model.

Pay: $42,000.00 - $52,445.50 per year
