WE’RE HIRING! CARE COORDINATOR / LIFE PLAN & OUTREACH SPECIALIST
Families First Community Center is growing, and we’re looking for an experienced, self-directed professional to join our team in Ellsworth!
This full-time position combines Behavioral Health Care Coordination, Life Planning, and community outreach supporting individuals and families experiencing homelessness or housing instability.
We’re looking for someone who:
- Has an MHRT-c or provisional
- Understands MaineCare, prior authorizations, and billing
- Is comfortable working independently and in the community
- Is organized, dependable, and a strong communicator
- Has a valid Maine driver’s license and reliable transportation
- Is passionate about working with vulnerable populations
Full-Time | Ellsworth, Maine
$45,000–$50,000 salary, commensurate with experience
