Fall River Health Services is hiring a Licensed Practical Nurse (LPN) to join our multidisciplinary care team in Host Springs SD! This position offers the chance to provide compassionate, high-quality care in a supportive and team-oriented environment.
About the Role
As an LPN, you’ll provide general nursing care to residents and play a key role in ensuring safe, effective, and person-centered care. You’ll work closely with the nursing team, physicians, and families to meet the unique needs of each resident.
Responsibilities

Provide nursing care following physician orders and LPN scope of practice.

Supervise and guide Nursing Assistants during assigned shifts.

Coordinate and oversee resident care plans in collaboration with the Director of Nursing or Charge Nurse.

Administer and chart medications and treatments accurately and safely.

Maintain complete and timely documentation to support continuity of care.

Communicate effectively with residents, families, and healthcare providers.

Assist with resident assessments, care planning, and evaluation.

Promote infection control and safety standards.

Participate in the Quality Management Program to ensure compliance with care standards.
Qualifications

Current Licensed Practical Nurse (LPN) license in South Dakota (or eligibility for reciprocity).

Strong communication, leadership, and clinical skills.

Experience in long-term care or skilled nursing preferred.
