What You'll Do
As a Case Manager, you'll work directly with youth and families in their homes, schools, and communities to help them build stability, strengthen relationships, and achieve their goals.
Your responsibilities will include:
Develop meaningful relationships with youth and families through a strength-based, trauma-responsive approach
Manage an assigned caseload while partnering with families, schools, funding agencies, and community providers
Provide mentoring, coaching, advocacy, and supportive services in homes, schools, and community settings
Connect families with essential resources including housing, food, utilities, healthcare, and other community supports
Help youth build life skills, improve social and emotional functioning, and participate in healthy recreational and educational activities
Coordinate services with treatment teams and community partners to ensure continuity of care
Develop treatment plans, document progress, and maintain accurate electronic records
Participate in supervision, professional development, and team meetings
Travel throughout the Cape Cod service area to meet families where they are
Promote the ARC (Attachment, Regulation, and Competency) framework in every interaction with youth and families

Qualifications
Bachelor's Degree
Experience working with children, adolescents, and families
Experience in human services, behavioral health, education, or social services
Bilingual candidates are strongly encouraged to apply
