RN Clinical Manager - Home Health

Location: Sarasota, FL
Schedule: Full-Time, primarily in-office with occasional field work
Compensation: $85,000-$89,000 base salary up to 10% performance bonus, paid quarterly

About the Role

Our client is an established home health organization seeking an experienced RN Clinical Manager to oversee clinical operations and support the delivery of high-quality patient care in the Sarasota, FL area.

This is a primarily office-based leadership position with occasional field involvement as needed. The RN Clinical Manager will work closely with clinical staff, field clinicians, and agency leadership to ensure patients receive appropriate, compliant, and coordinated care.

The ideal candidate is an experienced home health RN who understands the clinical and operational demands of a Medicare-certified home health environment and is comfortable balancing staff leadership, patient care oversight, documentation, and regulatory compliance.

Responsibilities

• Provide clinical leadership and day-to-day oversight of the home health clinical team
• Supervise, support, and mentor RNs and other field clinicians
• Review patient plans of care and ensure services are delivered according to physician orders and agency standards
• Monitor patient outcomes and identify opportunities to improve quality of care
• Review clinical documentation for accuracy, completeness, and regulatory compliance
• Support OASIS documentation and ensure appropriate clinical practices are followed
• Coordinate with physicians, therapists, caregivers, and other members of the interdisciplinary care team
• Assist with patient admissions, recertifications, resumptions of care, and discharges
• Monitor clinical caseloads and help ensure appropriate staffing and assignment of patients
• Provide guidance to clinicians regarding changes in patient condition, clinical concerns, and care planning
• Participate in quality assurance, performance improvement, and compliance initiatives
• Help maintain compliance with Medicare, state, federal, and agency requirements
• Utilize WellSky for clinical documentation, patient management, and workflow oversight
• Assist with onboarding, education, and ongoing development of clinical staff
• Provide occasional field support when operational or patient needs require it
• Help foster a collaborative, accountable, and patient-focused clinical environment

Qualifications

• Active and unrestricted Florida RN license
• Recent home health nursing experience is required
• Previous clinical leadership, supervisory, or management experience within home health strongly preferred
• Strong understanding of Medicare home health regulations and clinical documentation requirements
• Knowledge of OASIS and home health standards of care
• Experience with WellSky preferred
• Strong clinical judgment and problem-solving skills
• Ability to effectively coach, support, and hold clinical staff accountable
• Excellent organizational and time-management skills
• Strong written and verbal communication skills
• Comfortable working primarily in an office setting while providing occasional field support when needed
• Valid driver's license and reliable transportation

Compensation & Benefits

The RN Clinical Manager will receive a base salary of $85,000-$89,000, along with the opportunity to earn an additional 10% performance-based bonus paid quarterly.
