Faith Hospice has a unique opportunity for an Access Coordinator to join their amazing team on a full-time basis on-site at our beautiful main office in Byron Center!
What Will You Do as an Access Coordinator?
- Administratively support the Faith Hospice intake, admissions, and palliative care teams.
- Ensure team members are assigned to appropriate patients
- Review patient files to ensure they contain required documents; acquire any additional information required

Schedule patient sign-on, informational, evaluation, and admission visits for patients/families
- Assign visits to appropriate staff
- Manage admissions activities, schedules and calendar.
- Assist with electronic sign-on and education for patients regarding the admission process via phone, text and email
