Overview:

We are seeking a compassionate and skilled Psychiatric and/or Medical Nurse Practitioner to join our multidisciplinary team in delivering high-quality behavioral health and medical services. The ideal candidate will have a passion for mental and physical health care, experience in psychiatric assessments, medication management, and the ability to work collaboratively with a diverse team of healthcare providers.
Key Responsibilities:
- Psychiatric Assessment & Diagnosis: Conduct comprehensive psychiatric evaluations, including mental health history, symptom assessment, and risk evaluations for patients of all ages.
- Medication Management: Prescribe and monitor medications, adjusting treatment plans as necessary to ensure efficacy and patient safety.
- Collaborative Care: Work closely with primary care providers, therapists, and other healthcare professionals to create holistic, integrated care plans for patients.
- Patient Education: Provide patients and families with education regarding psychiatric or medical conditions, treatment options, and medication adherence.
- Crisis Intervention: Address acute psychiatric concerns, including emergency assessments, risk management, and referrals to inpatient or specialty care when necessary.
- Treatment Planning & Documentation: Develop individualized treatment plans, maintain accurate and up-to-date patient records, and document care in compliance with state, federal, and organizational standards.
- Cultural Competency: Provide culturally sensitive care and respect diverse backgrounds, ensuring equitable treatment for all patients, including those from underserved populations.
- Compliance & Regulations: Ensure that all services are provided in accordance with FQHC regulations, state/federal guidelines, and best practices for mental and physical health care.
Qualifications:
- Education: PMHNP or Doctor of Nursing Practice (DNP) with a specialization in Psychiatric Mental Health Nursing and or APRN License
- Licensure: Active and unrestricted Psychiatric Nurse Practitioner (PMHNP) license in Kentucky, Certification from the American Nurses Credentialing Center (ANCC) or equivalent.
- Experience:
- Minimum of 1 year of experience in psychiatric mental health or physical care preferred, preferably in a community health, FQHC, or outpatient setting.
- Experience working with diverse, underserved populations is highly preferred.
- Skills & Competencies:
- Strong knowledge of psychiatric and medical disorders, treatment modalities, and pharmacology.
- Excellent communication, interpersonal, and collaborative skills.
- Ability to work independently as well as part of a multidisciplinary team.
- Proficient in electronic health records (EHR) systems and clinical documentation.
- Strong organizational skills and the ability to manage a high-volume caseload.
- Cultural Competency: Demonstrated ability to work with diverse populations, including underserved, economically disadvantaged, and multilingual communities.
