We are seeking a compassionate, flexible Hospice Chaplain to join our dedicated interdisciplinary team on a per diem basis. This is an opportunity for a caring and spiritually grounded professional to provide meaningful support to patients and families as they navigate the end-of-life journey.
The Hospice Chaplain is responsible for the provision of spiritual care services to patients, families and caregivers of Faith & Family Hospice, either directly or through coordination of care with other spiritual counselors, offering spiritual and emotional support based on each patient's individual beliefs, values, and wishes. Working alongside and collaboratively with our full-time Chaplain, Nurses, Social Workers, Home Health Aides, Physicians, Music Therapists, and leadership team, the Chaplain helps ensure that the whole person-and the whole family-is cared for.
Responsibilities:
*Additional duties may be assigned in maintaining a bereavement program for families.
Qualifications: Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological degree). Experience working with death and dying, individuals, family, and caregiver. Ability to work and comfort in an interfaith setting. Ability to accept different lifestyles, cultures, beliefs, and values.
Pay: $30.00 - $35.00 per hour

Responsibilities:
- Assures spiritual assessment of patients and families and caregivers in the hospice program and appropriate services are rendered in a timely manner
- Provides direct spiritual care to patients and families and caregivers
- Attends patient care conference and interdisciplinary group meetings as a member of the interdisciplinary team
- Plans periodic memorial services to meet the needs of personnel, volunteers, and community clergy and spiritual counselors working with the hospice
- Document all interactions into the patient record by end of shift
