FAITH COMMUNITY HEALTH SYSTEM Jacksboro, Texas Nurse Practitioner JOB DESCRIPTION REPORTS TO: Medical Director SUPERVISES: None FLSA CLASSIFICATION: Exempt SUMMARY: Responsible for managing health problems and coordinating health care for the patient in accordance with State and Federal rules and regulations and the nursing standards of care (assessment of health status, diagnosis, development of plan of care and treatment, implementation of treatment plan, and evaluation of patient status). ESSENTIAL FUNCTIONS  Functions independently to perform age-appropriate history and physical for complex acute, critical, and chronically ill perioperative patients.  Orders and interprets pharmacologic and non-pharmacologic treatment modalities.  Implements interventions to support the patient to regain or maintain physiologic stability, including but not limited to serving in the first assisting role.  Monitors the effectiveness of interventions.  Facilitates the patients transition within and between health care settings, e.g. admitting, transferring, and discharging patients.  Collaborates with multidisciplinary team members by making appropriate referrals.  Facilitates staff, patient and family decision making by providing educational tools.  Duties, responsibilities and activities may change, or new ones may be assigned at any time with or without notice. PROFESSIONAL REQUIREMENTS  Adhere to dress code, appearance is neat and clean.  Complete annual education requirements.  Maintain patient confidentiality at all times.  Report to work on time and as scheduled.  Wear identification while on duty.  Maintain regulatory requirements, including all state, federal and local regulations.  Represent the organization in a positive and professional manner at all times.  Comply with all organizational policies and standards regarding ethical business practices.  Communicate the mission, ethics and goals of the organization.  Participate in performance improvement and continuous quality improvement activities.  Attend regular staff meetings and in-services as needed.

Decisions and problem solving require a combination of analysis, evaluation, and interpretive thinking.  Computer literacy, including but not limited to, data entry, retrieval, and report generation.  Must not have any restrictions for physical work for which reasonable accommodation cannot be made. QUALIFICATIONS  Graduate of an accredited school of nursing  Graduate of an accredited Nurse Practitioner Program  Current Advanced Practice Registered Nurse (APRN) Licensure  Current Nurse Practitioner certification in specialty area, if applicable  Current provider card in BLS  ACLS, PALS, and ATLS as applicable for position  Eligible for hospital privileges  Two (2) years’ experience in the Nurse Practitioner role preferred.
